Abstract

In 2030, Korea was announced to be the world's longest-lived nation by WHO and lancet recently. That is, the majority of customers coming to the pharmacy will be the elderly. Many people complain that pharmacy management is difficult these days, but the crisis is an opportunity. At this time, I would like to recommend studying about gerontology and Geriatric medication reconciliation. Elderly illness is manifested as a physical symptom with various problems such as loss of role in the society, solitude, nutritional status and economic status. In addition, their physical characteristics differ from those of their youth, thus, pharmacokinetics and pharmacodynamics are different. Most elderly people are suffering from chronic diseases and depression. Therefore, when we counsel them, many considerations such as adverse events, drug interactions, drug-drug interactions, drug-disease interaction, etc., must be considered. In this study, we will briefly review the pharmacokinetics, pharmacodynamics of geriatrics, and the role of pharmacists in elderly medication counseling. Even though it’s not easy to do it, the sense of fulfillment and rewarding as a pharmacist will be enormous when it comes to discovering inappropriate medication for the elderly, solving problems one by one, and providing practical help to elderly patients. Geriatric medication reconciliation is like puzzle matching.
